Use our contact form or call the practice. Share your concern, state of residence, and how to reach you. This is not for medical emergencies — call 911 or 988 if you are in crisis.

Practice reviews your message
Our team responds to discuss fit, licensing (NY, CA, or FL), and next steps. Submitting the form does not guarantee an appointment or specific outcomes.

Complete intake materials
If moving forward, you will receive secure intake forms covering health and psychiatric history before your first telehealth visit.

Attend your telehealth evaluation
Meet with Astrude Charles, PMHNP-BC, via HIPAA-compliant video. All care is telehealth only — no in-office visits.
