Medication management

Medication Management

Starting psychiatric medication — or adjusting a dose that isn't working — can feel overwhelming. You may wonder if side effects are normal, if it's helping, or whether you should keep going at all.

Medication management means you are never navigating this alone. Your PMHNP monitors your response, explains every change clearly, and adjusts your plan collaboratively over time.

Understanding your care

What is medication management?

Medication management is ongoing psychiatric care for patients on — or considering — psychiatric medications. It includes monitoring symptoms, reviewing side effects, and adjusting treatment when clinically appropriate.

This is not a one-time prescription. It is a relationship with the same PMHNP who tracks how you are doing visit after visit and helps you stay on a path that makes sense for your life.

Your PMHNP explains why a medication is recommended, what to expect in the first weeks, and when to reach out between visits. Decisions are made together — not handed down without context.

All medication management visits are conducted through secure telehealth for patients in New York, California, and Florida.

You are not imagining it

Symptoms you may be experiencing

If any of these feel familiar, you are not alone — and you do not have to figure out next steps by yourself.

  • New psychiatric medication and uncertainty about what to expect
  • Side effects that are uncomfortable, confusing, or worrying
  • Symptoms that improved initially but have plateaued or returned
  • Difficulty remembering to take medication or staying consistent
  • Anxiety about dependence, stigma, or long-term medication use
  • Needing dose adjustments after life changes, stress, or new symptoms
  • Multiple medications and wanting a clinician to review the full picture
  • Stabilizing after a psychiatric evaluation and needing ongoing follow-up

How it works

How treatment works

A clear path forward — so you always know what happens next.

  1. 01

    Establish your baseline

Your PMHNP reviews your current medications, symptoms, and history — often following an initial evaluation or records from a prior provider.

  2. 02

    Set clear goals

Together you define what improvement looks like: better sleep, less anxiety, improved focus, or steadier mood.

  3. 03

    Monitor and adjust

Regular telehealth follow-ups track your response, side effects, and functioning. Adjustments are made when clinically appropriate.

  4. 04

    Maintain stability

As symptoms stabilize, visit frequency may adjust — but your PMHNP remains available for changes in your life or symptoms.

Why treatment helps

Benefits of medication management

  • Ongoing monitoring so you are not adjusting medication alone
  • Side-effect guidance and dose changes when clinically appropriate
  • Same clinician who knows your history visit after visit
  • Electronic prescriptions when permitted by law
  • Flexible telehealth follow-ups from home
  • Honest discussion when medication is not the right fit

Is this right for you?

Who is this for?

  • Patients starting psychiatric medication after an evaluation
  • Those on existing medications needing monitoring or adjustments
  • People experiencing side effects or plateaued symptom improvement
  • Adults transferring medication management from another provider
  • Anyone wanting a PMHNP relationship for long-term psychiatric care

Before you worry

What to expect during your appointment

  • Follow-up visits are typically 20–30 minutes, focused on how you have been feeling since your last appointment.
  • Your PMHNP asks about symptom changes, side effects, sleep, appetite, energy, and daily functioning.
  • If an adjustment is recommended, you will hear why — including benefits, risks, and what to watch for.
  • Prescriptions are sent electronically when clinically appropriate and permitted by law.
  • You can ask questions at any point. No concern is too small if it is affecting your willingness to stay in treatment.
  • Visit frequency depends on your needs — more often when starting or adjusting, less often when stable.
